GIVE-Africa Home Nursing Program Strategic Plan (2025–2027)
Our Vision
To deliver high-quality, compassionate, and accessible nursing care to vulnerable populations, enhancing health outcomes and fostering resilience within refugee settlements and surrounding communities.
Our Mission
To strengthen home-based nursing services by integrating Family Planning, Mental Health and Psychosocial Support (MHPSS), and other supportive health programs. Using mobile clinics, on-call services, and strategic partnerships, we aim to create a holistic care ecosystem.
Strategic Objectives
- Expand Home Nursing Coverage: Launch services in Lwamwanja (2025), Kyangwali (2026), and Nakivale (2027) refugee settlements.
- Enhance Integration: Incorporate Family Planning and MHPSS services into nursing visits.
- Improve Service Delivery: Transition to home-based care with expanded mobile clinics and on-call services.
- Strengthen Referral Networks: Streamline connections with health centers and NGOs.
- Increase Community Impact: Focus on chronic disease management and reduce facility dependency.
Key Milestones
2025: Lwamwanja Refugee Settlement
- Serve 300 patients monthly in Kyaka II.
- Launch mobile clinics and conduct outreach campaigns.
- Partner with local health centers and NGOs.
2026: Kyangwali Refugee Settlement
- Serve 600 patients monthly in Kyaka II.
- Strengthen Family Planning and MHPSS integration.
- Optimize multi-settlement mobile clinic logistics.
2027: Nakivale Refugee Settlement
- Serve 100 patients monthly in Kyaka II while scaling new settlements.
- Introduce advanced chronic care management.
- Expand sports and First Aid programs for engagement.
Implementation Framework
2025: Staff recruitment, launch mobile clinics, and evaluate initial progress.
2026: Expand to Kyangwali, host MHPSS training, and strengthen referral pathways.
2027: Start services in Nakivale, enhance chronic care, and publish an impact report.
